Patient Activation through Community Conversations (PACC)

PACCs are a safe place to have a discussion about being a patient, an advocate or a caregiver*
When I was asked to visit a woman in the nursing home who cried that she wanted to die, her daughter said this was not "normal" behavior. When we went through her medications, with an independent pharmacist, it turned out that her medications were causing this behavior.

How do you keep track of your medications and medical care? Do you get copies of your test results? Learn what works for others at the May 4th PACC.

No lectures, just a conversation!
Many of us end up as patients or caregivers at some point without the skills to get through the complex healthcare system. Now we can help each other.
 
What is your question and how have others handled a similar situation? 

This virtual group meets the first Wednesday of the month and participation is free.
 
Don’t expect to get medical advice here, rather learn how others might handle the same situation. Find out where you can go for help.

Why TakeCHARGE?

The TakeCHARGE Campaign promotes “5 Steps to Safer Health Care” so people who use the healthcare system can be better prepared.

Patient advocates are encouraged to help people prepare, and those people who are contemplating becoming patient advocates can use TakeCHARGE to help support the community in preparing. There will be a meet-and-greet for TakeCHARGE Campaign participants on the first Monday of each month from April through September to share ideas, experiences, articles and more on how they are participating in
TakeCHARGE.
The 5 Steps:
  1. Understand and Complete Your Advance Directives
  2. Keep a Record of Your Medical History and Current Medications
  3. Prepare for Doctor Visits/Make a List of Questions
  4. Prevent Infections/Ask Caregivers to Wash Their Hands
  5. Use an Advocate/Be an Advocate for Others
