Oops!  How to Fix a Logging Error
Use the Edit Service Log Wizard
 
It is easy to delete or change a log in the Service Portal.  You can find logs that have yet to be exported under the Edit Student Service Log Wizard  When you enter the Edit Wizard, click on Search to see your most recent logs.  Select Edit or Delete next to the log to make changes to an individual log.
Notify the SWMSS office if there is an error on a log that has already exported to DPHHS.


You can also delete a non-billable log, note or absence.  This is done from the Service Log Wizard.  Select the student and choose a date from the same month as the error on the calendar.  The next screen will take you to the service type screen where you would record an absence.  At the bottom of this screen is a list of the service logs and non-billable notes and absences for the student.  Click on the trash can next to the record you wish to delete.

The Student Information Sheet

The Student Information Sheet has gotten an upgrade. We have made the form fillable so you can now type in the information needed to add a student to the Service Portal.  You can still print out a blank form and fill it in.  To remain HIPAA and FERPA compliant, please don't send the completed form as an email attachment.

In some districts, the completed form is given to the Special Education office to send to SWMSS.  Providers can also save the completed form as a PDF and scan using the SWMSS secure link, fax, or mail it to the SWMSS office.
