N95 Mask Distribution
CHDC will be participating in the HRSA Health Center COVID-19 Mask Program. We are waiting for our shipment and are not yet distributing the N95 masks. We will update this page with more information; please check back often. No phone calls please.
COVID-19 Vaccines
CHDC is pleased to offer both Moderna and Pfizer Covid vaccines at our Coventry Mall location. We offer 1st, 2nd, 3rd and booster doses for patients 12 years and older. Boosters are available 5 months after the completion of your Covid vaccine series and your vaccination card is required upon scheduled appointment. To schedule an appointment, please email covidreg@ch-dc.org or call 484-941-9991.
In addition, CHDC offers Pfizer Pediatric Covid-19 vaccines for children ages 5-11 during a scheduled pediatric office visit. If the child is not a CHDC patient and but is interested in receiving a Covid vaccine, we periodically hold Covid-19 vaccine clinics at our Coventry Mall location. Your child will need to be registered for a clinic at our health center and an appointment will be made at that time. Stay tuned for announcements regarding upcoming pediatric vaccine clinics.

Healthy Smiles Learning Program for Kids
Wednesday, February 23

Spring City Library
In this free 15 minute program, kids will learn more about teeth, healthy foods, and how to practice good dental habits! Kids will receive some fun dental freebies and a free book about healthy smile habits. No registration needed. We hope to see you there!

Diabetes Prevention Program

9 out of 10 people with prediabetes do not know that they have it. We are working with The National Diabetes Prevention Program to help prevent you from developing Type 2 Diabetes. Specially trained health coaches will work with you and teach you how to eat healthy, be more physically active, and manage stress.

On Friday, 2/5, CHDC staff celebrated Wear Red Day for American Heart Month! Whether in the office or working remotely, our staff is proud to show their support for women's heart health and raise awareness about cardiovascular disease.
Here at CHDC, we care for your heart by helping you manage risk factors for heart disease such as diabetes, high blood pressure, high cholesterol, inactivity, and more! The best way to fight heart disease is to have a healthy balanced diet, regular exercise, and make sure you’re getting regular check ups with your doctor.

Congratulations to our Care Management patients who have lowered their A1C! When working with a patient to lower their A1C, the goal is to manage their prediabetes to prevent Type 2 diabetes from occuring. At CHDC, our Care Management team will work with patients, healthcare providers, and families to help patients accomplish their health goals.

Focusing on your heart health has never been more important. Heart disease is the leading cause of death in the United States. But you can do a lot to protect your heart. Self-care is heart-health care. Practicing self-care can keep our hearts healthy. Being physically active, eating healthier foods, getting enough sleep, not smoking, and finding healthy ways to reduce stress can help prevent heart disease. And, when we take care of our hearts, we set an example for those around us to do the same.

Each February, the American Dental Association (ADA) sponsors National Children's Dental Health Month to raise awareness about the importance of oral health. Developing good habits at an early age and scheduling regular dental visits helps children to get a good start on a lifetime of healthy teeth and gums. We want to encourage healthy habits from a young age, such as brushing, flossing, eating healthy snacks, and having routine dental visits.

CHDC Expands Dispensary Services with Pottstown Pharmacy Partnership
Community Health & Dental Care, Inc. and Pottstown Pharmacy have entered into a relationship to expand pharmacy services and medication discounts to patients in the Pottstown and surrounding communities.
CHDC patients can conveniently get their prescriptions filled either at the CHDC dispensary at the Coventry Mall, 351 W. Schuylkill Road, Pottstown 19465 or at Pottstown Pharmacy, 1501 East High Street, Pottstown 19464.
Pottstown Pharmacy also offers FREE DELIVERY service within our service area. Eligible patients who need access to medication at an affordable price- especially higher costs meds – can receive medication discounts. Eligible patients must be an established medical patient of CHDC and complete the required application to determine healthcare discount to be applied. Eligible patients applying for CHDC’s healthcare discount program may receive medications for as low as $5.00 a prescription. Referrals welcome.

Qualified Medicare Beneficiaries Can Save Thousands
CHDC's Case Management team will help Medicare recipients navigate the Medicare registration process and understand the Medicare buy-in program which helps eligible individuals pay for certain out-of-pocket healthcare costs. Individuals do not need to be a CHDC patient to take advantage of this free service. Seniors with limited income and resources may qualify to have their Medicare Part B premium paid for, saving thousands of dollars by receiving maximum benefits from the Medicare program. Medicare beneficiaries are encouraged to call and make an appointment with our Case Management team at 610-326-9460 ext. 117
