Beginning June 1,2023, policyholders will need to complete the Request for Funds Withdrawal form (located on the website and agent's portal) for all withdrawal from: Cash Value, Dividend Accumulations, Surrendering Paid-Up Additions, Annuity, Supplemental Contracts, Pre-Paid Premium Deposits and PPE Accounts. In addition, the FedEx fee will be $30 and if they require a signature, it will be an additional $7.
