Mental Health and Disabilities Survey

Hello,

 

I am writing to let you know about an opportunity to participate in a voluntary research study about mental health supports for adults with disabilities in Utah. This study is being conducted by the Institute for Disability Research at Utah State University and has been approved by the USU Institutional Review Board (Protocol #12468).


As part of our data collection, we are conducting a survey to assess the availability of mental health supports for individuals with disabilities. We are seeking a broad range of respondents, consisting of individuals with disabilities, family members, mental health providers, and disability service providers including direct support staff.

Survey responses are anonymous and cannot be connected to your identity. The information you share will help guide our project planning efforts. This survey should take about 10 minutes to complete.
