  • Alexander Smith, our Care Pathways Lead, recently took the stage at the RISE Population Health Management (PHM) OHT Collaborative to share valuable lessons learned from implementing a PHM approach to Integrated Care Pathways.

Proposed designation requirements posted for public comment


The Ontario government is proposing a new regulation under the Connecting Care Act, 2019 (CCA) to establish additional requirements for the designation of Ontario Health Teams (OHTs).


The ministry has posted a draft regulation of proposed designation requirements on the Ontario Regulatory Registry for public comment. All health system stakeholders and partners are encouraged to review the proposed requirements and may submit written feedback by April 8, 2024. Comments received during the consultation period will be considered in the final preparation of the regulation.

OHT Data Dashboard release: Primary care


The OHT Data Dashboard: Primary Care report, (March1, 2024) provides an overview of key metrics such as visits to primary care practitioners, virtual care visits, specialist visits, and service utilization, and offers insights into healthcare patterns of attributed populations. Feedback is welcome at ohtanalytics@ontariohealth.ca.

Secure Messaging Proof-of-Concept pilot


The ministry has initiated a two-year Secure Messaging Proof-of-Concept (PoC) pilot for Ontario physicians to enhance patient-physician communication, improve access to care, and collect evidence on secure messaging in practices. Physicians in the pilot can claim public funding for eligible uninsured services delivered through secure messaging. Registration is now open until July 31, 2024, with the PoC running from April 1, 2024, to March 31, 2026.

ST JOSEPH'S HEALTH CARE LONDON - ROAD TO MENTAL HEALTH (R2MH) PROGRAM

The Road to Mental Health (R2MH) program is a free service connecting youth in rural Middlesex County & First Nations communities to mental health and addictions care in London. All drivers for crisis calls have received training on mental health, crisis de-escalation and suicide prevention.


Providers can access this program to provide free taxis for their clients by contacting melissa.taylor-gates@sjhc.london.on.ca to receive instructions. Youth can also access this program directly by contacting Reach Out at 1-866-933-2023 or www.reachout247.ca.

SCHULICH SCHOOL OF MEDICINE AND DENTISTRY - INVITATION TO LEARN ABOUT SIMULATED PARTICIPANT OPPORTUNITIES

Clinical Skills Learning Program (CSLP) at the Schulich School of Medicine & Dentistry, Western University is recruiting people to be Simulated Participants (SP) and are hosting a virtual townhall for people to learn what it means to be an SP.


This session will cover the critical role SPs play in their educational endeavors, the process of becoming an SP, and the impact you can have on the next generation of medical and dental professionals. It's also a great opportunity to ask questions and learn more about the program directly from their team.

SELF-MANAGEMENT PROGRAMS NETWORK

An online directory of self-management workshops in Southwestern Ontario is now available. The Self-Management Programs Network website lists upcoming self-management workshops in our area for patients, caregivers, and healthcare providers.  

 

These workshops share skills that can help you manage your health condition and make your life better. They also teach you how to support someone you care about with their health condition. Examples of workshops include "Living a Health Life with Chronic Pain” and “Powerful Tools for Caregivers”. 

 

Programs are also available for Healthcare Providers that promote working in partnership with patients and caregivers to achieve better health outcomes.
