PROJECTS APPROVED
Following extensive planning and review by our Operations Team, Coordinating Council approved us to immediately proceed with several projects which will have the greatest impact on patients, clients, caregivers or healthcare providers. The projects and a brief summary are listed below.

  1. Care Manager Model - Foundational work to understand what should be included in the model.
  2. Equity, Diversity, Inclusion (EDI) Collaborative - To regularly connect EDI Leads/Groups to efficiently work together on shared priorities. If you or a colleague would like to participate, please email info@mloht.ca.
  3. Individualized Care Plan - Foundational work to examine best practices
  4. Let's Go Home Initiative - Supports providers and patients by coordinating care and removing non-clinical barriers to discharge from hospital (e.g., to fund access to meals, transportation)
  5. Virtual Care Maturity Model - Conduct an environmental scan to gauge current state of virtual care operations across Middlesex London to determine priorities and targets.
PRIVACY EXPERTS NEEDED – OHT PRIVACY ADVISORY COMMITTEE
The Middlesex London Ontario Health Team is looking to create a Privacy Advisory Committee to assess and review privacy issues related to our upcoming projects and PHIPA (Personal Health Information Protection Act) legislation changes.

The committee would develop documents/templates or processes to ensure current and future projects understand the implications of privacy issues and help to identify privacy challenges. Interested individuals do not need to have a legal background, but should be comfortable in privacy ethics and identifying trouble areas.

The committee would meet once per month for one hour, along with 1-2 hours of preparation time.

As more aspects of healthcare becomes digital, it’s important to have privacy-related matters as a pillar of our work. For example, if a project intends to use patient health information, what questions should be asked and understood during project planning?

WELCOME TO THE MLOHT OPERATIONS TEAM
We are excited to welcome Rachel LaBonté as our Practice Facilitation Lead, and Honey Shaikh as our Business Analyst to the MLOHT Operations Team.

Rachel will help to identify and break down barriers to assist clinics and organizations to implement health system improvements. She joins us August 18th from Ontario Health West and the Partnering for Quality Practice Facilitation Program.

Honey joins us August 8th and will analyze, document, interpret and present data to advance Population Health Management.
SURVEY: PHYSICIAN FEEDBACK ON GERIATRIC SERVICES
Specialized Geriatric Services at St. Joseph’s Health Care London and the South West Frail Senior Strategy are looking for feedback from primary care physicians to better understand the referral process when caring for older patients. The goal is to improve the referral form and the centralized coordinated intake process.

eREFERRAL SURVEY COMING TO AN INBOX NEAR YOU
Since the eFax integration to Ocean eReferral was launched on May 24, 148 referrals were sent electronically via eFax in Middlesex London.

The average monthly volume of eReferrals in our region has increased by more than 115% (from 941 referrals/month to 2,048 referrals/month) since work started on this project in December 2021. Great work! Together we can Axe the Fax!

We want to hear about your experience with Ocean eFax. On August 15, all eReferral senders and eFax recipients will receive a survey by email and fax, respectively. We deeply appreciate every member of your team for their efforts in this initiative.

Congratulations to Mohamed Daher, the MLOHT Digital Health Project Manager.

Mohamed recently graduated from the Schulich School of Medicine and Dentistry - Masters of Public Health Program, Class of 2021. The graduation ceremony last month was delayed from October 2021 due to the pandemic.

The program is a 12-month, case-based learning program and we are proud to recognize Mohamed on this incredible achievement.

Our PCCPC is looking for new members who want to share their experiences and ideas on how to transform and improve our health care system.

ENGAGEMENT GUIDE LAUNCHED BY STRATEGIC DIRECTION OFFICE
To make community engagement easier, the London Middlesex Mental Health and Addiction Strategic Direction Office recently launched an Engagement Guide to help in engagement planning, implementation, and evaluation.  
 
“Anyone who wants to engage with people with lived experiences can use this,” says Knowledge Mobilization Specialist Tanvin Juneja. “It can assist different organizations with varying degrees of engagement experience from those who are new to community engagement to those who are familiar with it.”
 
The guide can be used by all organizations and is not limited to mental health and addictions teams.
 
The easily downloadable guide has been used for a variety of purposes from designing waiting rooms to creating family advisory councils.  It was developed following extensive consultations with people with lived experience, system leaders and service providers. 
 
It includes four sections: an introduction, an easy-to-use 16-point checklist, information briefs associated with each checklist point, and fillable workspaces.
