REFLECTIONS FROM COORDINATING COUNCIL JUNE 23, 2022

We need to remember the importance of being kind to ourselves and to each other. During a difficult time of change in health care and high stress due to the pandemic, we need to prioritize boundaries to take care of ourselves, our loved ones, our colleagues and patients/clients. When we are kind to ourselves, we can be kind to others.
Some Physicians use a screening tool called Adverse Children Experiences (ACEs) to examine how a person's past and childhood experiences can impact how a person responds to a specific stressful situation. It is not diagnostic, but may indicate a need for further investigation. Learn more.

MLOHT OPERATIONS TEAM RETREAT
|
Our MLOHT Operations Team came together in person for the first time on Wednesday, June 8 for a full day retreat to set our priorities in population health management and integrated care. It also helped us to identify gaps and opportunities in several ongoing projects and possible new initiatives moving forward.
Prior to this, our work had been done virtually so the face-to-face working time was a welcome change and a great opportunity for team building.
Based on the feedback from the retreat and Coordinating Council, we will complete the Population Health Management and Equity Plan to be submitted to the Ministry of Health on July 29, 2022. It will serve as the MLOHT's Operations Plan.

SURVEY: VISION FOR INTEGRATED HEALTH CARE

As we continue to make progress toward a more integrated health care system, it begs the question 'what will be different?'
Following our MLOHT Retreat, we asked Coordinating Council some thought-provoking questions which sparked an interesting discussion on the vision for integrated care and how it will benefit patients/clients and health care providers.
We ask you to take the short survey by Friday, July 22 and answer those questions as we identify gaps and opportunities to be addressed.

On June 14, 2022, LMPCA Executive Council's Dr. Vineet Nair, Dr. Eric Zhigang Tian of eHealth Centre of Excellence and Primary Care Physician Dr. Scott Laing moderated a virtual town hall to discuss and demonstrate the benefits of eReferrals, how Ocean eReferral is used in EMRs, how eReferrals via eFax is used in Ocean and how to enter appointment information to provide patient notifications about upcoming referrals.
The Middlesex London Ontario Health Team partnered with Ontario eServices Program and London Middlesex Primary Care Alliance to host the town hall.
At last count, more than 100 local specialists have been added to eReferral.

LONDON DOCTOR APPLAUDS eREFERRALS
eReferrals via eFax was launched on May 24, 2022 in Middlesex London and according to one local physician, it is helping to make his practice more efficient.
"The success of the Referral Management Program and eReferrals relies heavily on the number of physicians and specialists who sign on. The more who sign on, the better," says London Family Physician and owner of The Core Family Health Centre Dr. Vineet Nair. "Referring patients using Ocean ereferral/efax has recently shifted the practice in my clinics. Since I started using Ocean ereferrals, things became easier for me, my team and my patients by reducing administration time at the physician level and improving communication via emails with patients. Today, 50% of my referrals are done through Ocean.”
The project has highly supported the transition toward the full eReferral solution in Middlesex London.

Congratulations to our very own Mulugeta Chala, MLOHT Impact Fellow who received his PhD from the School of Rehabilitation Therapy at Queen's University at a June 22 convocation.
His thesis focused on self-management of chronic low back pain in Ethiopia studying adaptation, understanding the lived experience of people with chronic low back pain and exploring the country's health care providers' understanding of self-management support. He hopes to use his research to develop culturally appropriate chronic low back pain self-managements programs.

CONSOLIDATED SELF-MANAGEMENT WEBSITE PRESENTATION

Our team, Hanadi Al-Sadek of the Middlesex London Ontario Health Team and Sally Boyle of Ontario Health West, shared the successes and presented the benefits of having a consolidated website for self management workshops for both patients/clients, caregivers and healthcare providers.

PATIENT/CLIENT & CARE PARTNER COUNCIL SEEKS NEW MEMBERS
|
|
Our PCCPC is looking for new members who want to share their experiences and ideas on how to transform and improve our health care system. The PCCPC works in partnership with the Coordinating Council and together they play a key role in supporting and being accountable to patient/client and caregiver health system governance.
We encourage you to please print and post the attached poster in your offices and clinics to help promote the PCCPC as it looks for new voices who have had experiences in our health care system.
If you know someone who may be interested in being a member of this valuable group, feel free to share the poster with them.

HEALTH CARE NAVIGATION PLANNING PROJECT TEAM IS NOW SET

Our Health Care Navigation Planning Project Team has been formed and is eager to get to work to provide patients/clients, caregivers and care partners with 24/7 navigation supports.
The team consists of experts from diverse backgrounds who will add great value to the project. Members of the team are:
- Chantel Antone (Indigenous Program Specialist/Navigator, South West Regional Cancer Program, London Health Sciences Centre)
- Jacobi Elliott (Evaluation Lead in Specialized Geriatric Services at St. Joseph’s Health Care and Co-Design Lead with the MLOHT)
- Nancy Hines (Retired Social Worker and Caregiver and PCCP Council Representative)
- Craig Olmstead (TVFHT Family Physician at Parkhill Medical Clinic and an Adjunct Professor at the Department of Family Medicine at Western University)
- Alexander Smith (Process Design and Change Management Lead at London Health Sciences Centre and Care Pathways Lead with MLOHT)
- Summer Thorp (System Facilitator, London Middlesex Mental Health and Addiction Strategic Direction Office)

The team will work together to establish 24/7 navigation supports by September 2023, to align with the provincial Health Care Navigation Service (HCNS) to make service navigation easier and more equitable for everyone in the community.
