WHOLISTIC NEEDS PRE-SCREENER: HELP US TO MANAGE OUR POPULATION HEALTH AND SOCIAL NEEDS

The key to a successful and effective healthcare system is insight into the main health and social needs of the population. London Health Sciences Centre, in partnership with the Middlesex London Ontario Health Team, would like to invite you to take part in a co-design session for a Wholistic Needs Pre-Screening tool.

The goal of the pre-screener is to enable a more preventative approach to managing individual’s health and social needs and to connect people more easily to appropriate services. We are looking for community members and service providers to be a part of the process.

We would like to thank everyone who participated in our previous co-design sessions! These sessions have led to some great discussion and next steps. There is still the opportunity participate in co-design sessions or 1:1 meetings.

For further information, questions, or to participate, please reach out to: Laura Cuthbertson

EMPOWERING HEALTH CARE WORKERS

For questions, please email Lindsey Moore.

PRIMARY CARE RECRUITMENT

Data from 2022 estimates that over 65,000 residents of Middlesex London do not have access to a primary care provider. Middlesex London is not attracting enough primary care providers to provide primary care to its growing population, let alone replacing the retiring providers. A project is in progress to develop a primary care recruitment, transition into practice, and retention program for Middlesex London.

We are holding a kick-off meeting on May 25th with key stakeholders to look at how we can collectively support the development of this program.

For more details or if you'd like to be included in this meeting, please reach out to Melissa Linseman

CALL FOR CO-DESIGN PARTICIPANTS: METHAMPHETAMINE PATHWAY

In partnership with the London Middlesex Mental Health and Addiction Strategic Direction Office and London Health Sciences Centre, we are working on a project to develop care pathways for people who use methamphetamine or are at-risk of using methamphetamine.

We need your help. We are looking for people who interact directly or indirectly with people who use substances to join the co-design network to inform and guide the development. These care pathways will allow people who use methamphetamine to receive better care and not fall through the cracks.

First Session: Wednesday, May 17, 2023, 12:30pm-2pm.

Second session: Tuesday, June 13, 2023, 2:00pm-3:30pm.

The time commitment is flexible and based on your availability.

CANADIAN HEART FAILURE AWARENESS WEEK

The 5th Annual National Heart Failure Awareness Week, organized by the Canadian Heart Failure Society, focuses on the theme "It's not normal to be breathless." The week-long campaign aims to raise awareness about breathlessness as a common symptom of heart failure, and the importance of early detection and management of the condition.

Through a range of activities and educational resources, the campaign aims to educate Canadians about the warning signs of heart failure and the lifestyle changes and medical interventions that can help prevent and manage the condition. By promoting early detection and treatment, the Canadian Heart Failure Society hopes to improve the quality of life for those living with heart failure in Canada.

RED DRESS DAY

Red Dress Day, also known as The National Day of Awareness for Missing and Murdered Indigenous Women and Girls and Two-Spirit People, is an annual event held in Canada on May 5th to honour the lives of missing and murdered Indigenous women, girls, and two-spirit individuals.

This day serves as a call to action for Canadians to acknowledge the systemic violence and discrimination faced by Indigenous women, girls, and two-spirit individuals and to take steps towards reconciliation and healing. Through this event, we can remember those who have been lost and continue to work towards a future where Indigenous people can live free from violence and oppression.
