The MLOHT recently hosted an Information and Engagement session to talk about some of the great projects and initiatives we are working on to improve health care experiences for patients, clients, care partners and providers. The goal was to inform, engage and connect with our valuable partners and stakeholders to advance our common purpose of health system improvement.
The session was recorded and is available to anyone who might not have been able to join or simply wants to revisit the content.

PATIENT, CLIENT, CARE PARTNER COUNCIL OUTREACH | |
Do you know a group or organization that could benefit from learning about our Patient, Client, Care Partner Council (PCCPC)?
The PCCPC held its first outreach event October 27th when the group attended the London Health Sciences Centre (LHSC) Patient Experience Advisory Committee (PEAC) Retreat to present its work on how it is striving to move past the status quo, and improve the patient, client, and caregiver experience.
"It did clarify several questions in my mind," wrote Kirk Patterson who attended the presentation. "As I mentioned before, by stating 'sign me up', I want to reiterate that if you need another Patient Partner, I would be glad to be the link from our council to yours."
The PCCPC will make similar presentations to other Patient, Client, Family, Caregiver tables across the MLOHT region.

25 AGENCIES ATTEND INAUGURAL NAVIGATOR COLLABORATIVE | |
Nearly 40 people, representing 25 agencies, attended our first meeting of the Middlesex London Navigator Collaborative to raise collective awareness of the resources available to help patients, clients, caregivers, care partners, and providers.
The ultimate goal of the collaborative is to help patients/clients find the appropriate help they need, and it starts with providers learning about community resources and tools that can help.
The feedback following the meeting was positive and a few connections were made to help their patients/clients.
“Great meeting, I think it went well and everyone is jazzed up about the information sharing and rapport building between agencies," said one of the participants.
At each meeting, three agencies share a brief overview about their organization and services. The group meets every other month.

MLOHT PRESENTS AT THE FIRST PROVINCIAL INTEGRATED HEART FAILURE CARE COMMUNITY OF PRACTICE MEETING | |
It was a team presentation as the MLOHT and Huron Perth and Area OHT co-presented at the inaugural Integrated Heart Failure Care CoP to discuss the state of our current projects and challenges.

PRAISE FOR OUR POPULATION HEALTH MANAGEMENT AND EQUITY PLAN | |
The West Region System Strategy, Planning, Design and Implementation (SSPDI) Team and provincial partners congratulated us following our submission of our Population Health Management and Equity Plan.
It observed effective work in advancing an Equity, Diversity, and Inclusion Collaborative, relationship- building in local First Nation communities, the Self-Management West Region consolidated website, our partnerships with other OHTs, our PCCP council, a focus on initial and full population , and COVID-19 response.

The London Middlesex Primary Care Alliance is hosting a town hall on Thursday, November 10, 2022 from 7pm to 8:30pm on eConsult and eReferral in your EMR.
It will include practical Ocean demonstrations in Telus and Accuro by Dr. Vineet Nair, Dr. Daniel Pepe, and Dr. Eric Tian.

PRIMARY CARE PHYSICIAN ENGAGEMENT RESEARCH | |
Researchers at Western University are looking for primary care healthcare workers to take part in a study to explore the success of the London Middlesex Primary Care Alliance and primary care physician engagement.
Participants will take part in a 30-minute Zoom interview with results to be published in peer reviewed journals.
The goal of the study is to help shape physician engagement in the region.

Population Health Management Office at LHSC | |
London Health Sciences Centre recently opened its new Office of Population Health Management and Health System Partnership to support the work to build a more connected health care system to help patients and families.
A key to this success is building and strengthening health system partnerships.
The new office demonstrates LHSC's commitment to health care transformation and will focus on five key areas:
- Health system coordination and governance
- Care design
- Change management
- Population level data, analytics and technology
- Research, innovation and education
