INVITATION TO CO-DESIGN COPD FUNDING OPPORTUNITY | |
We are reaching out to system partners to join an exploratory discussion on an integrated submission for funding inclusive of our regional hospitals, primary care, home and community care, and other health and social services sectors. We are hosting two engagement sessions November 20 and November 22 to co-design an integrated care project and accompanying proposal. It is critical to have as many individuals and diverse perspectives join these discussions to ensure we are building an integrated care model that meets the needs of our local system.
These sessions will build off of one another, so if possible, please try to have organizational representation at both sessions. Please register for the engagement sessions here:

We're thrilled to announce that Middlesex London OHT has been selected as one of the 12 Accelerated Ontario Health Teams!
We are proud of this major milestone in our journey to transformation and are honoured to be among the 12 OHTs selected to advance our maturity at an accelerated rate, based on the following domains: Governance, Clinical Service Transformation/Population Health Management, Leadership, Readiness for Home Care Transition, Primary Care Engagement & Leadership, Delivering an Integrated Clinical Pathway and/or Home Care Leading Project, and Strong Back Office.
We are committed to advancing our maturity and to sharing our experience with all Ontario Health Teams as we focus on the following Accelerated OHT Priorities:
- Implement clinical pathways
- Improve system navigation
- Home care readiness
- Implement supports for people without connection to primary care
- Improve early detection/intervention for chronic disease
- Creating a Not-for-Profit Corporation
- Establishing or aligning a Primary Care Network (PCN)
- Selecting an Operational Support Provider (OSP)

IMPROVING ACCESS TO VACCINES AND INTERPRETATION SERVICES | |
We have received funding of over $43,000 from the Alliance for Healthier Communities to continue offering On-Demand, Virtual, Interpretation Services at Primary Care Clinics. This free virtual interpretation service can be used for any typeof appointment, as long as the topic of vaccine is included in the discussion.
This funding provides us with an amazing opportunity to improve access to vaccination and improve access to health care conversations in a person's language of comfort. Over the past year, the program has supported more than 580 health care conversations. We would like to expand this offering of virtual, on-demand interpretation services to more people.

MIDDLESEX LONDON OHT: PROJECT HIGHLIGHTS | |
ONLINE APPOINTMENT BOOKING (OAB) | |
We are thrilled to share some fantastic news with you regarding our ongoing efforts to enhance healthcare services in our region. Ontario Health has allocated funding for 212 Primary Care Online Appointment Booking licenses for the Middlesex London OHT, and we are making significant progress in putting these resources to good use.
As of the latest update, we have successfully supported activation of 70 of these licenses, and they are now live and accessible to our valued patients. This means that over 35,000 individuals in our community now have the convenience of booking their primary care appointments online, streamlining the process and providing a more efficient and user-friendly experience.

Overview
Grounded Roots, Open Wings (GROW) provides comprehensive pre- and post-natal care to those in our community who do not have a family doctor. In addition, we provide wrap-around care to the majority of our service users who deal with socioeconomic and psychosocial challenges.
We are seeking dedicated and passionate individuals to join our Board of Directors for a three year term. Board members provide governance, financial and strategic oversight to advance the mission of GROW. We currently meet once a month, with a mix of in-person and videoconferencing.
Key Considerations
We are looking for candidates with strong leadership and strategic skills, a good understanding of the organization's mission and vision and the ability to communicate it effectively. Specific skills of interest this year include:
- finance, and knowledge of the legal and regulatory environment
- experience in fundraising/development
- grant writing
- governance experience
If you have the skills and experience to advance the work of GROW, we would like to hear from you. GROW is committed to broadening the diversity of our Board of Directors and encourage applications from individuals with a variety of lived experiences.
