The goal of the Consolidated Self-Management Website was to remove geographical barriers to increase access to workshops for all - it has succeeded!
On March 31st, 2022, the Self Management Programs Network was launched to bring together all self-management workshops from the four sub-regions, within Ontario Health West, into one centralized website to maximize program offerings, and improve access to and awareness of self-management programs.
By eliminating the geographical barriers between South West, Waterloo-Wellington, Hamilton-Niagara-Haldimand-Brant and Erie-St. Clair sub-regions, residents can easily browse self-management workshops to determine the best program that suit their timing and needs.
In the first three months of the website from April-June, close to 800 people from the different sub-regions registered for a workshop.
Of the 800 participants, 75% resided in the sub-region, 9% out of the sub-region but within Ontario Health West and 16% came from outside of the Ontario Health West Region.
“This proves that when you eliminate hurdles, residents will take advantage of the offerings and discover other programs to help themselves,” said Sally Boyle, Manager of System Strategy, Planning, Design and Implementation for the South West Self Management Program. “These numbers speak for themselves. We are pleased with the results thus far.”
The website offers people living with a chronic illness, caregivers and providers all the self-management programs available in the Ontario Health West region in one central location.
