Pediatric Celiac Disease Survey 

The Pediatric Gastroenterology team at the Children's Hospital LHSC are conducting a survey to better learn the practice patterns and understanding of primary care providers related to pediatric celiac disease and the gluten free diet.


As part of this process, LHSC are conducting an anonymous survey of primary care providers, which will require approximately five minutes to complete. It will remain open until early fall 2025. Click here to access the survey.


New and Improved Long-Term Care Assessment Form

The Ministry of Long-Term Care has launched a modernized Health Assessment Form, which is used to complete the health assessment required by law when a person applies for long-term care. 


Through consultation and collaboration with users and key partners, including the OMA, the form has been updated to clarify some questions, be easier to complete, and better address the information needs of all partners.

Online Appointment Booking funding Now Available

Ontario Health has announced one-time funding for 2025/2026 to support the implementation of Online Appointment Booking for new applicants and existing users. If your clinic is interested in accessing this funding, please complete the Expression of Interest no later than Thursday, August 21st. Click here for more information.

Co-Design Summary Report: Access to Primary Care

On April 24, the Middlesex London Ontario Health Team and the MLPCN brought together community members and health-care partners for a collaborative co-design session aimed at expanding access to team-based primary care across Middlesex London.


Centered on the Health Home Model, the session focused on creating equitable, community-rooted care for all. This report highlights key insights from the event, including priorities identified through in-depth discussions on health equity, team roles, enabling infrastructure, and quality improvement.
