Continuous Quality Improvement (CQI) Toolkit

Download this new resource to help your organization or team learn effective implementation strategies that drive meaningful, lasting improvements to work processes!

About the toolkit:

CQI requires consistent dedication to monitoring services and taking action to improve them. Today’s mental health and human service settings often have limited fiscal and staffing resources. Day-to-day service delivery requirements often take priority over CQI efforts, resulting in a reactive model that responds to crises rather than actively managing quality. These tools have been gathered to support teams of direct care staff, supervisors, and managers in your efforts toward an efficient and continuous approach to quality. Using these tools will help build a culture of quality. This practice will ensure a fulfilling experience for your organization’s workforce and the best outcomes for the people you serve.

The CMHIS Sustainment Planning Infographic outlines 10 practical steps that mental health programs can take to develop, implement, and refine a sustainment action plan—from defining what you want to sustain, to selecting sustainment strategies, to evaluating and continuously improving your sustainment efforts. This infographic helps lay a foundation for sustainment planning by highlighting the key components that support long-term program impact, even after a grant ends or funding changes.

Sustainment Planning Guide

Sustaining mental health services includes funding, but so much more! The CMHIS Sustainment Planning Guide is designed to help you develop a practical roadmap for sustaining your program or practice. It walks you step-by-step through creating, implementing, and refining a sustainment action plan with a worksheet for each step. The guide also includes an example of a sustainment action plan with a completed set of worksheets.
