Dear CIN Colleagues,
We are writing with an important update regarding Mount Sinai’s participation with Molina Healthcare and Affinity by Molina Healthcare (Molina). We are ending our participation with Molina, and Mount Sinai Doctors faculty practices, hospitals, and other facilities will be out of network with Molina starting Thursday, January 1, 2026. This is not a contract negotiation; rather, it is an organizational decision to end our participation with all Molina plans we participate in: Medicaid, Child Health Plus, HARP, and Essential Plan.
While the MSHP CIN does not contract with Molina, some of you may have individual contracts with Molina and refer us those patients. Mount Sinai will continue to care for established Molina patients (who have been seen within the past 12 months) until we are out of network on January 1, 2026. However, starting Sunday, June 1, 2025, Mount Sinai will not be scheduling any new Molina patients, so we ask that you do not refer new Molina patients to care at Mount Sinai starting June 1, 2025. A new patient is anyone who has not been seen at Mount Sinai within the prior 12 months (since June 1, 2024).

- EP 4 – Molina Healthcare NY Essential Plan 4
- EP 5 (200 – 250) – Molina Healthcare NY Essential Plan 5
As always, emergency care is not affected. All Molina patients will still have access to our Emergency Departments regardless of our network status.
We will communicate with Mount Sinai’s Molina patients about this termination later this fall and will keep you informed as we complete the process of ending our participation with Molina.
