Thank you to everyone who joined the Charting the Course meeting on December 2nd.


It was wonderful to gather some new faces with current NC MHI partners to share insights from different lenses. We were able to come together to define our 'North Star' in postpartum care, identify specific measures we can use to monitor our progress, and talk about the available resources and supports for postpartum care and how they connect and complement each other. Below, you can see graphic summaries of our shared space.

Postpartum Toolkit in Year 3



Last year, each site selected small change projects to improve postpartum care for their patients. In development for this year, we have created two pathways to assist sites in implementing small change projects. Sites can now choose between the Postpartum Preparation Pathway or the Quality Postpartum Visit Pathway. We hope to continue to refine and implement tools developed in previous years. Support Recruiting New Clinics


Join us! The 4th Trimester Project team at UNC-Chapel Hill has support from the HRSA-funded NC Maternal Health Innovation Initiative to collaborate with North Carolina clinics to improve care for postpartum people and their families. We are actively seeking 10 sites (clinics or health care systems) across all perinatal regions - to continue testing and implementing components of the North Carolina Postpartum Toolkit. The toolkit is modeled on ACOG's Postpartum Bundle but built for our state. We also hope to build a new community of thought leaders and early adopters in improving the postpartum care experience!

January 27th NC MHI Community of Practice
