Evidence of Insurability for Group Term Life
During Open Enrollment last fall, if you chose an amount of Term Life insurance that was above the guaranteed issue amount, you should have received (or may be receiving in the near future) a form to complete for Evidence of Insurability (EOI).
Please note, once an you receive an EOI form, this can be completed at any time. Although it says you have 60 days, if it is completed after 60 days it will still be considered. However, if a medical exam is required, this has to be performed within 30 days of the EOI completion, or another EOI will have to be done.
Therefore, if an you do not feel comfortable at this time going out of your home or having someone come into your home to receive this exam, it is recommended that you wait to complete the EOI until a later date.
Keep in mind, the pending Life insurance amount will not go into effect until all required steps of the EOI have been completed and approved by Voya.
