What You May Receive from NCFlex Plans
Below is information regarding the NCFlex plans and materials you may receive or have access to after your enrollment, and be sure to check each benefit section on ncflex.org for certificates, how to file claims, flyers on bonus benefits, and more.
Flexible Spending Accounts (FSAs)
- A convenience card is mailed to first-time users. For those with both Health Care FSA and Dependent Daycare FSA accounts, only one card is issued. Cards are re-issued only if reported lost or stolen, or once the card expires after three years. The date your card is valid through is listed on it. For security, please note these cards come in plain envelopes with minimal markings.

Additional cards can be requested by logging onto ncflex.padmin.com or by calling 1-866-916-3475. Note that a claims kit will be emailed to all participants and mailed to those without a valid email address on file with P&A. A claims kit can be found in the FSA section of ncflex.org.
Dental
- Cards with a unique ID will be issued to new employees who enroll and will be re-issued to existing participants only if you transfer to a new job or make changes to your plan. If you do not receive your cards or need additional cards, you can contact MetLife at 1-855-676-9441.

Generic cards without a unique ID can be printed after logging onto mybenefits.metlife.com using NCFlex as the company name. You can also find a generic card on NCFlex's website in the dental section. Cards are only required the first time you visit your dentist or if you change dentists.
Vision

Cards are only mailed to new participants. Additional cards can be accessed by logging onto eyemedvision.com/NCFlex, using the EyeMed app, or by calling 1-866-248-1939. ID cards are not required to visit a provider.
Accident, Critical Illness, AD&D* and Group Term Life*

Certificates, claim forms, and other information can be found in each plan's section of ncflex.org. ID cards are not provided for these plans and are not required.
- If you requested a Term Life amount for you or your spouse that requires Evidence of Insurability (EOI), you should receive correspondence from LifeHelp/Voya on how to complete this information via email. If no email address is on file, the information will be mailed to your address on file.
- Postcards for the Accident, AD&D*, Critical Illness, and Group Term Life* plans will be mailed to new enrollees for 2026.
Cancer

Welcome letters are mailed to new participants and anyone who makes a change to their coverage during Open Enrollment (OE). Certificates, claim forms, and other information about these plans can be found in the cancer section of ncflex.org.

The Standard (formerly Allstate) also has a portal where you can login to view your coverage and claim information. ID cards are not provided for this plan and are not required.
Disability*

There are no mailings for this plan. However, plan details and vendor contact information can be found in the disability section of ncflex.org.

EOI was required during Open Enrollment for anyone who enrolled for the first time after declining the plan when initially eligible. If EOI was not completed electronically during OE, you were contacted with instructions on how to continue. If you have not received a notification and still need to complete EOI, contact The Standard at ncflex@standard.com.
*Employees of the University System of North Carolina are not eligible to participate in the NCFlex AD&D, Group Term Life, or Disability Plans.
| | Have You Filed Your Wellness Benefit? | | |
Participants of NCFlex's Accident and Critical Illness Plans, administered by Voya, are eligible for a wellness benefit. This benefit is payable once per year, per covered person for completing an eligible health screening test.
The deadline to file a wellness claim through Voya is six months after the screening or at the end of the plan year, whichever is later. If you completed an eligible health screening in 2025 over 6 months ago, December 31, 2025 is your last day to file for this benefit.
If you participate in both plans, you can receive a wellness benefit from each plan for the same screening. Voya does not require documentation for its wellness benefits. You will only need information about yourself or the dependent you're filing for, date of service, and the provider's name. NCFlex's Cancer Plan, administered by The Standard (formerly Allstate), also offers a wellness benefit. This benefit is payable once per year, per covered person and does not have a filing deadline, but documentation is required. Access your account by visiting mybenefits.allstate.com.

| | Be Sure to Spend Your Flexible Spending Account Funds | |
Remember that your deadline to spend funds from your 2025 Health Care Flexible Spending Account (HCFSA) is December 31, 2025. Any funds over $640 will be lost on January 1, 2026.
If you have at least $25 in your account, up to $640 will roll over to the new year. Additionally, you don't have to be enrolled in the HCFSA for 2026 for 2025 money to roll over and remain in your account.
For those with a Dependent Day Care Flexible Spending Account (DDCFSA), your deadline to spend funds from 2025 is March 15, 2026. This plan has a grace period instead of a rollover, so all funds remaining in the account on March 16, 2026 will be lost.
Note: For expenses incurred before each FSA's respective deadline, you have until March 31, 2026, to file 2025 claims. This includes any out-of-pocket expenses paid for without your convenience card.
