TO ALL CAREGIVERS:
As a care provider, if you believe that your client needs higher level of care, there is a process the CMA needs to follow. HAR is our bible and all of us need to adhere to the rules. There is an Annual re-certification of LOC thru 1147. Although, we don't have to wait for the re-assessment month / year. If there is a significant change in the client's medical and physical condition, as a result of hospitalization or injury, we can do it. Here's the process for us to follow: Please send a written notice to the office. Fax documents (MD orders, H&P, progress notes, etc) that can support your request. The Monitoring RN will bring a copy of 1147 form during their monthly visit. (You can not call them to come in for a special trip.) Together, you will fill up the form 1147. If there is a discrepancy on your answer, you as the PCG need to justify your answer. You need to give examples and provide proof. After completion of the form, MD will review the form. Signed document will be submitted to the respective HMO. They will determine the appropriate LOC. NOTE: Being incontinent does not make the client Level 2.

REMINDER: Adverse Event Submissions

  1. Use the most current form from DHS: Form 519 Revised 09/14. (Click link below)
  2. Verbal report must be made within 24 hours and written report submitted within 72 hours.
  3. Fill in all the spaces that you are required to fill in including the dates and times of the event. 
  4. Do not write the client social security number in the Medicaid # blank. Only the medicaid number should be written. If client is private write private or medicaid # N/A(not applicable).
  5. Vital Signs should be taken and included in the report.
  6. If your client goes to the emergency room (ER), wait for the disposition of your client whether the client was sent back home or admitted before you send your written report. Write the ER or admitting diagnosis. Submit the ER summary together with the AE report.
  7. If your client is admitted to hospice, submit report related to change in condition. Hospice clients who sustain pressure injuries, falls, or other adverse events must be reported. When the hospice client dies, this must also be reported.
  8. Notify the family/guardian and write the date and time of notification. Write the name of the family/guardian. Indicate if client has no family/guardian or tried to contact but no response.
  9. Notify the physician and indicate the date and time. Write physician action taken/orders as a result of the adverse event or change in condition.
  10. Fax all reports and ER summaries to 380-4729.
