Greetings!
We are wrapping up the first quarter of the calendar year and are excited to highlight the wonderful work that has been happening in our workgroups along with sharing a few requests from our co-chairs. Our next all-member meeting is Friday, May 23rd at 1 pm MT.
Data Technology and Standards (DTS)
The NMSDOH Collaborative's Data & Technology workgroup is launching this form in an effort to build off of the Gravity Project's work and recommending that the HCA use SDOH domains for the referral system, which will require the least amount of transition for those transitioning into the system. The use of domains is instead of a standard screener for instance. However, for some organizations who do not already have a screener at all in place, they may have to adopt one. In this form, we are seeking input on a set of base domains to present as a floor for those to start with for an easy transition. Since the Gravity Project has done much of this work in defining and coding domains- their work is being used as the starting block. Access the form here.
Closed-loop Referral Systems (CLRS)
The CLRS workgroup reviewed results from their survey which focused on identifying which HRSN screening assessments are being used by Collaborative members. Results are available here.
The HCA is also welcoming your input regarding the statewide closed-loop referral system. At this time, HCA and Findhelp invite you to share organizations that you recommend be prioritized in the rollout of the CLRS. You can share your input using this excel sheet or submit suggestions directly to Roger at rstremming@findhelp.com.
Community Engagement (CE)
The CE workgroup is continuing to work on an annual survey and interview guide that will help measure and identify opportunities to strengthen the NMSDOH-C's impact.
Operations
The Operations workgroup will have a conversation about funding at the next meeting.
Tribal Health Systems (THS)
The THS workgroup is continuing to build out the Indigenous Social Determinants of Health Learning Series which will be available this fall. The THS workgroup worked on the activities and description for Session One at our last meeting.
Steering Committee Updates
The Steering Committee approved the logic model, and all workgroup chairs presented updates.
The Steering Committee also invites all voting member organizations to consider signing the NMSDOH-C Pledge. This pledge is a way to formally affirm our shared commitment to addressing the social drivers of health through collaboration, equity, and community-led solutions. By signing, you or your organization will:
- Demonstrate your commitment to advancing equity and justice
- Support meaningful community-driven collaboration
- Help strengthen systems that promote health and well-being for all
