July All-member Meeting

We look forward to seeing you at the all-member meeting on July 25th at 1 pm MT. During this meeting members will have the opportunity to learn more about the Member Pledge and review initial findings from the Measuring and Strengthening our Impact Survey. We do have time for one member presentation. If you are interested in presenting about your organization's work to address SDOH, please email dlucero45@phs.org.

Data Technology and Standards (DTS) and Closed-loop Referral Systems (CLRS)

The DTS and CLRS workgroups have developed a recommendation regarding screening tools and practices as the HCA continues its work to launch the statewide closed-loop referral system, Yes New Mexico Connect.

Community Engagement (CE)

The CE workgroup launched the "Measuring and Strengthening our Impact Survey". The survey is available for current voting members to complete at the following link. Please complete the survey by July 11th.

Operations

The Operations workgroup has drafted a Fund Development Plan for the Collaborative. The plan provides an overview of our approach to fundraising and outlines sustainability goals.

Tribal Health Systems (THS)

The THS workgroup has identified the following dates for the Rooted in Resilience: Healing the Circles within Indigenous Social Drivers of Health Co-learning Series. The sessions will take place on the following dates:

Session 1 - An Introduction to Indigenous Social Determinants of Health and the Systems that Impact Them In-person - Friday, October 17th 9:30 am - 2:30 pm 

Session 2 – Community-led Approaches to Integrating ISDOH within the Healthcare System - Virtual - Friday, October 24th 10:00 - 11:30 am

Session 3 – Using the ISDOH framework to Strengthen Assessment Virtual - Friday, October 31st 10:00 - 11:30 am

Session 4 – Committing to an ISDOH Framework Virtual - Friday, December 5th 10:00 - 11:30 am
