Welcome to INTERACTion, the Nassau Queens Performing Provider System’s (NQP) newsletter specifically focused on DSRIP project 2.b.vii–Implementation of the INTERACT® (Interventions to Reduce Acute Care Transfers) project.

NQP has been working closely with skilled nursing facilities (SNF) to implement the INTERACT program–a quality improvement program that focuses on the management of acute changes in a SNF's resident’s condition in order to stabilize the resident and avoid transfer to an acute care facility. This newsletter is meant to keep our SNF partners up-to-date on service and quality outcome measures specific to the INTERACT project, while also keeping all partners informed of our progress.
Skilled Nursing Facility PAC Meeting
The Nassau Queens Performing Provider System hosted its first in-person Skilled Nursing Facility (SNF) Workgroup and Quality Committee INTERACT and PAC meeting at Mercy Medical Center in Nassau County on January 30, 2018.

Topics of focus included:
  • NQP’s SNF Quarterly INTERACTion Newsletter,
  • SNF performance reporting dashboard
  • Quality reporting of re-hospitalization & QAPI data tracking
  • Use of RHIO/HIE and other forms of electronic clinical data, quality reporting of re-hospitalization, Workforce Strategy and training collaboration with GNYHA and the Resource Material - 2018 MLTC VBP quality measure

Stern Family Center Launches Telemedicine Program
The  Northwell Health Stern Family Center for Rehabilitation  has launched a telemedicine program to provide clinical bedside support using two-way video communication to assess and diagnose clinical problems and minimize hospital readmissions at the 256-bed subacute care facility. Read more.
Townhouse Center for Rehabilitation and Nursing Implement Realizes Success Using INTERACT Tools; Grateful to NUMC Hub
The NUMC Hub's Joyce Nolasco's, Clinical Liaison Post-Acute/Long-Term Care, NUMC Hub , and Renee Wasserman's, Clinical Liaison Post-Acute/Long-Term Care, NUMC Hub , work with the Townhouse Center for Rehabilitation has resulted in a decline in hospitalizations for the facility through the implementation of INTERACT tools. Additionally, Townhouse went from a 1-star to a 2-star facility, and increased to a 4-star facility on Quality Measures. Read the letter of appreciation from Townhouse's Administrator, Meredith A. Data Analytics Overview
Opportunity for improvement
Through data analysis of Skilled Nursing Facility DSRIP Quality Measures, NQP has determined that there are opportunities for improvement in working towards reducing preventable ED visits. The data shows that 70% of the Skilled Nursing Facility population have preventable ED visits.

Where we are thriving
Our data analysis indicates that we are very successful in achieving medication adherence metrics including antipsychotic medication adherence, diabetes screening (antipsychotic medication), antidepressant medication management (acute), and antidepressant medication management (cont.).
