Welcome to the first edition of NQP Pulse, the newsletter of the Nassau Queens Performing Provider System (NQP). This newsletter is meant to keep you up-to-date on relevant activities, events, and resources surrounding DSRIP and NQP.

Nuhealth-Nassau University Medical Center Sees Success with Transitions of Care Project – DSRIP Project 2.b.iv
Through their efforts surrounding the DSRIP Transitions of Care project, Nuhealth has seen a significant decrease in readmissions for their chronically ill patients, highlighting their success with congestive heart failure (CHF) patients. Since project implementation, 35% of CHF discharges have not resulted in a readmission, as compared to the national average of ≥50% of CHF discharges resulting in a readmission within 6 months of discharge.  Read more

St. John’s Episcopal Hospital Integrates Behavioral Health and Primary Care
St. John’s Episcopal Hospital, an NQP partner through Catholic Health Services of Long Island, is participating in DSRIP Project 3.a.i (Integration of primary care and behavioral health services), and has recently incorporated a behavioral health specialist, Enny Almonte-Nunez, LCSW, into its primary care practice at 495 Beach 20th Street in Far Rockaway, NY. The practice’s primary care physicians are now able to provide patients with a warm handoff to an on-site behavioral health specialist, making behavioral health counseling easier to receive for those patients who could benefit from it the most.  Read more

Medical Record Abstraction Process
Partners selected for the Medical Record Abstraction review will be notified and contacted by the NYS DOH medical record review vendor, MedReview Inc., to begin scheduling appointments for record collection services. The medical record abstraction process applies to measures such as screening for clinical depression and follow-up, controlling high blood pressure, and comprehensive diabetes care. The results from the reviews determine our final performance which is linked to funds flow payment to the PPS.

Value-Based Payments Corner
As part of DSRIP goals, all managed care organizations (MCO) must employ non-fee-for-service payment systems that reward value over volume for at least 80% of their provider payments by the year 2020 (waiver Year 5).

To help NQP partners understand value-based payments (VBP), the VBP Corner will provide information regarding our efforts towards promoting value and quality of care.  To start, NYSDOH has created a Value-Based Payments (VBP) for Providers video. This video highlights how VBP will reshape our health care system and the important role providers play.  Click here to access the video.

Licensed Behavioral Health Practitioner (Off-Site Clinic) Benefit Guidance
NYSDOH recently released a document containing guidance on the Licensed Behavioral Health Practitioner (LBHP) benefit offered under NYS’s 1115 Waiver. This benefit permits Medicaid Mainstream Managed Care Organizations (MMCOs) and Health and Recovery Plans (HARPs) to reimburse Clinic Treatment Programs licensed by the NYS Office of Mental Health for the provision of services at locations other than the clinic’s main site(s) or satellite locations. Under this new managed care benefit, OMH clinics (licensed under 14 NYCRR Part 599 and 598) can now be reimbursed by MMCOs for an array of services which can be provided off-site to both children and adults.
