As a courtesy, we periodically provide informational emails. This one is regarding the NYS Workers' Compensation Board change to the C-11 form.
The NYS Workers' Compensation Board has announced the updated the C-11 form to include an “X” designation for those who do not wish to identify as male or female.
Please discard any previous versions prior to the June 2022 update. We ask that you begin using the new version now. Here is the link to access and print the new form: Workers' Compensation Board C-11 Form.
