COVID-19 Vaccination Updates

In addition to offering the first and second dose of the COVID-19 vaccines, CHDC will also be administering third doses to those who qualify. Individuals who are moderately or severely immunocompromised can get the third dose, at least 28 days after the second dose. For everyone else, CDC recommends getting the third dose 8 months after the second dose. Pfizer vaccines are available for individuals ages 12+, and Moderna vaccines are available for individuals 18+. In order to receive the third dose of the vaccine, you must bring your COVID-19 vaccination card as this will be used to determine your 3rd dose date. Please note you may not receive your 3rd dose if you forget your vaccine card. Please schedule your appointment using the email or phone number below.
Covid-19 vaccines are available at CHDC for everyone age 12 and over! To schedule, please email covidreg@ch-dc.org or call 484-941-9991. Protect yourself and others against Covid-19. Please continue to wear a mask, wash your hands, practice social distancing, and get vaccinated.

Community Health Centers: The Chemistry for Strong Communities

Thank you to all who participated in our National Health Center Week Celebrations at CHDC this year! While we weren't able to have some of the events that we had hoped for due to the ongoing pandemic, we were still able to celebrate both in-person and virtually!
Community Health Centers serve more than 28 million patients in the United States, with more than 917,000 of those being Pennsylvanians. Community health center workers are on the frontlines of the COVID-19 pandemic, caring for some of the hardest hit communities. We could not have continued to serve our 37,000 registered patients throughout the pandemic without the support of our community.

Public Health in Housing Day
To kick off NHCW 2021, our team created a Social Determinants of Health Screening (PRAPARE) educational brochure. It has been distributed to staff and patients at our locations. As we educate the community about SDoH, this brochure will be a useful tool in answering some commonly asked questions about PRAPARE.
This information not only helps us understand each of our patients, but gives us the tools to be a better advocate for our community.

Healthcare for the Homeless Day
Agricultural Worker Health Day
In the spirit of Health Center Week, our staff donated an entire car full of items that will be distributed to homeless individuals in Pottstown! Throughout the past two weeks, staff have brought in individually packaged food items, drinks, feminine products, reading glasses, toiletries, gift cards, and more. These items will be distributed by the Street Medicine Program, which works to provide primary and urgent care to those experiencing homelessness in Pottstown Borough wherever they reside- whether that be a tent in the woods, a homeless shelter, or on the street.

Patient Appreciation Day
So often, we are inspired by the people who walk through our doors, and feel lucky to be a trusted healthcare provider to our community. Our patients are at the heart of everything we do; check out our Patient Appreciation Day video to hear it from us!

Our Physical Therapy team helped organize and host an awesome scavenger hunt for CHDC patients and community members. It was a great indoor activity for a very hot August afternoon. Everyone who stopped by was able to grab a pedometer, snacks, CHDC goodies, and prizes. Be on the lookout for more events like this in the future!

Staff Appreciation Day
We celebrated our staff this year with a staff lunch and gift during our monthly meeting. Throughout the week, we invited our young health center patients to color in a "Healthcare Heroes" coloring page, which we have proudly hung up! Children's Health Day
Throughout the week at all of our locations, we had the opportunity for patients to enter a free raffle! The three baskets were filled with fun themed items for kids. We had a sports basket, a games & crafts basket, and a back-to-school basket. Congratulations to the winners!

"Take What You Need. Leave What You Can."

This simple sign holds some very powerful words. This food pantry, strategically located in front of 414 High Street Pottstown PA 19464 at Mission First Church, is only a few feet from the sidewalk. Most people who would see a refrigerator sitting under a little house outside, in front of a church, may do a double take. That is the goal in order to bring awareness to the food insecurities within our homeless community.
Mission First and the Recovery Learning Center (located inside of the church) is a place often visited frequently by our homeless population throughout the week. Mission First is also one of the main stops for the Street Medicine Team. Patients who are homeless within our community in need of on-the-spot medical attention and get connected to other resources such as primary care (CHDC), help with housing or shelter, get their medication(s), behavioral health, and much more.
This food pantry is designed to be easily accessible to anyone who may need it, and conveniently located to place donations. Click the link to learn more about the food pantry, such as what items are most needed, and how to participate.

CHDC Mobile Oral Health Unit is on the Move!
We have been hard at work preparing this space to serve our community members who need it most. Our providers are ready to offer much needed dental services to patients throughout the Tricounty area. This space has everything our providers will need to perform dental exams, dental recalls, sealants, fillings, x-rays, and simple extractions! Our mobile team, consisting of a hygienist, dentist, dental assistant, and a dental mobile unit coordinator are ready to serve patients of all ages.

CHDC Expands Dispensary Services with Pottstown Pharmacy Partnership
Community Health & Dental Care, Inc. and Pottstown Pharmacy have entered into a relationship to expand pharmacy services and medication discounts to patients in the Pottstown and surrounding communities.
CHDC patients can conveniently get their prescriptions filled either at the CHDC dispensary at the Coventry Mall, 351 W. Schuylkill Road, Pottstown 19465 or at Pottstown Pharmacy, 1501 East High Street, Pottstown 19464.
Pottstown Pharmacy also offers FREE DELIVERY service within our service area. Eligible patients who need access to medication at an affordable price- especially higher costs meds – can receive medication discounts. Eligible patients must be an established medical patient of CHDC and complete the required application to determine healthcare discount to be applied. Eligible patients applying for CHDC’s healthcare discount program may receive medications for as low as $5.00 a prescription. Referrals welcome.

COVID-19 Health & Safety Precautions

At Community Health and Dental Care, we are committed to a safe and clean environment as the COVID-19 pandemic continues. Locations have been triple-cleaned, all staff are wearing masks, and seating in lobby and waiting rooms are socially distant.

Octodent equipment have been installed in our dental facilities, and patients are being scheduled to allow for appropriate levels of social distancing in our facility. COVID-19 pre-screening includes temperature checks of all patients upon arrival for their scheduled appointment. All patients and staff are required to wear masks.
