It’s Easy to Make Enrollment Changes

QualChoice gives you choices for making changes to your group enrollment or selecting new benefit plans for members.

• Change Form: The enrollment Change Form is available at QualChoice.com, under Find a Form or Document. 
• Electronic Spreadsheet: If you prefer to submit a spreadsheet of enrollment changes, your Account Service Representative (PDF) can provide you with an electronic template. This is an easy way to send updates for several members at once or to add benefit plan offerings at renewal.
• The My Account Portal: If you haven’t already registered for My Account access, now is the time! Making enrollment additions and changes here is fast, easy, and secure. Use the My Account Portal to enroll new hires, make year-round changes online, view eligibility details, enter Open Enrollment selections on renewal, and more. ACA Affordability Percentage for 2025

The Internal Revenue Service (IRS) Revenue Procedure 2024-35 recently updated the Affordable Care Act (ACA) affordability percentage to 9.02% for 2025 from 8.39% in 2024. The affordability percentage determines which applicable large employers (ALEs) may face penalties under ACA §4980H(b) for failure to offer affordable coverage, as well as which individuals may qualify for subsidized coverage through a public Exchange.
