New! Electronic Invoice Delivery in My Account
As part of our ongoing efforts to provide our valued brokers with the highest level of service, QualChoice routinely reviews processes for ease and efficiency. The following change will help improve the timeliness of invoices and reduce overall costs: Effective January 1, 2026, QualChoice will no longer send premium invoices via mail or email.
All employers will be automatically enrolled for electronic delivery of invoices, with the ability to view all premium invoices through the My Account portal. All invoices will be available to view on the portal on or after the 15th of each month for the next month’s billing cycle.
Only those who elect to opt out will receive a mailed invoice. We will no longer offer email invoices as an option.

In order to process your opt-out request prior to the first billing cycle of 2026, please return this form via email, fax, or mail to us no later than December 13, 2025. If your request is received after December 13, 2025, the change to receive a paper form will go into effect for the next billing cycle following our receipt of your request.
