We are pleased to offer a new way for you to submit a request for consultation: a secure online form. Here are a few details to keep in mind prior to submitting your online consult request:
The questions on the form are very similar to the questions your Behavioral Health Consultant (BHC) asks you by phone. The form could take up to 10 minutes to complete, depending on the complexity of the patient information.
You may submit the form anytime, but your consult will be completed during our business hours, 9 am to 5 pm, M-F, excluding holidays.
Upon receipt of your online consult request your regional BHC may contact you if additional information is required.
