In 2017, Heartland has a new wellness website, where you can check to see if Heartland has received your Wellness  Passport, learn if you’ve earned your incentive, access tools, resources, support and more.
Visit heartlandwellnessfund.com on your computer, tablet or phone, click on the blue Wellness tab and set up  your account to explore. (You will need the last six digits of your social security number to set up your account.)

Understand Your Results
A registered nurse from the HealthLink team may reach out to you to assist with understanding your Biometric screening results. HealthLink nurses can answer any questions you may have and provide resources  for managing and improving your results and your health. In addition, the nurse can provide assistance in locating a primary care physician in your area if you do not have one to assist you with any follow-up services needed on your path to wellness. This assistance is a benefit of Heartland Health & Wellness Fund, so please consider taking advantage of this service

There are many ways to get and stay healthy. Ever wonder if your hard work is paying off? Heartland's new Wellness Website offers a health assessment to give you your Overall Wellness Score. Once you know your score, the Wellness Website gives you tools and resources to improve your health:

• Health trackers to help you track your nutrition, exercise and weight management 

• Wellness calculators to measure BMI, calorie burn, nutrition and more

• Action guides to help you plan and reach your wellness goals

• Personal to-do lists to organize your wellness activities
