Medicare Advanced Primary Care Management (APCM) Program

No Cost plus Additional Personalized Care

We are excited to introduce our Medicare Advanced Primary Care Management (APCM) Program, launching this January. This program is part of an innovative initiative by Medicare to support primary care providers in delivering more comprehensive and coordinated care. By offering new opportunities for managing complex care needs through a value-based approach, the APCM program allows us to focus on providing you with high-quality, personalized care that prioritizes your overall well-being.

Participation in APCM is designed to enhance your Medicare benefits by ensuring you receive the right care, at the right time, in a way that aligns with your unique needs and goals. Please let us know that you would like to participate, by sending a portal message to Dr. Saltzman who will be overseeing the program, and email to our Quality Manager, Maria Daly at mdaly@rwwc.com.

It’s important to understand the details of participating in this program:

  1. APCM services are available to you, but only one practitioner can furnish these services during a calendar month.
  2. You have the right to stop these services at any time, effective at the end of the calendar month.
  3. Standard Medicare cost-sharing may apply.

By agreeing to participate, you are also acknowledging that your care team will assume responsibility for all your primary care services and serve as the ongoing focal point for all your healthcare needs. While written consent is not required, your agreement to participate will be documented in your medical record.

We are thrilled to offer this new program and look forward to partnering with you on your journey to better health. Together, we can ensure that you receive the personalized, proactive care you deserve!

Pneumonia Vaccine For Patients 50+

The CDC has updated their recommendations, and now recommend that all adults, 50 and older receive the pneumococcal vaccine.

Pneumococcal bacteria can cause serious illnesses, including pneumonia, meningitis, and bloodstream infections, and older adults are at increased risk for pneumococcal disease.

If you would like to schedule an appointment for the vaccine, please call us at 215-735-7992, send a message to your provider on the portal, or email us at info@rwwc.com.
