Travel Assistance Grant (TAG) Program funds up to $15,000 a year for patient & caregiver travel.

What is TAG?
As the top destination online for OM-related information, OMF is inundated with requests from around the world for assistance with research, treatment and travel. We do our best to fulfill these requests, but we have for some time wished to have a more structured program for disbursing funds specifically for patient travel. We feel this is critical to reducing the friction of getting patient in front of the experts with whom they so desperately need to interface.

Enter TAG.

Taking a cue from Mercy Medical Airlift and Luke's Wings, a popular Washington, DC charity which flies military families to visit wounded warriors, OMF is pleased to announce the Travel Assistance Grant (TAG) program to provide needs-based travel assistance to patients and their immediate caregivers.

Commitment to Patients
OMF is committing up to $15,000 per year for individuals and their immediate caregivers to travel by air, rail or car to see the uveal melanoma experts of their choosing.  

Applications for TAG Awards will be evaluated solely on need. The anticipated award is $500 per trip with a maximum family gift total of $1,500.

How do I apply?
Requesting travel is easy. 
Applicants can submit an online application via the OMF website or download an application form and mail it in. We will need proof of income/financial status along with any supporting materials.
Again, we're extremely excited to roll this exciting program out and see it touch the lives of patients and caregivers across the country.
