Guidelines for Patient-Centered Documentation in the Era of Open Notes: A Qualitative Study
The study "Guidelines for Patient-Centered Documentation in the Era of Open Notes" (Vanka et al.) identified ten evidence-informed guidelines to support clinicians in crafting notes that empower patients and mitigate harm. Through focus groups with patients, physicians, and medical educators, the research offers practical strategies to improve medical documentation, emphasizing respectful language, transparency, and person-centered care.
