What are Loss to

Follow-up and

Loss to Documentation?

Loss to follow-up is when a baby does not pass their newborn hearing screening, and does not receive additional follow-up (e.g. rescreening, diagnostic hearing evaluation).

IN THIS ISSUE

What is Loss to Follow-up and Loss to Documentation?

Promising Practices for Preventing Loss to Follow-up

Loss to documentation occurs when results from a newborn hearing screening or rescreening are not reported or are reported inaccurately. In this issue of Newborn Hearing Screening Monthly, we provide some promising practices to prevent loss to follow-up. Practices to prevent loss to documentation will be addressed in September’s issue of the NBHS Monthly.

Promising Practices to Prevent Loss to Follow-up

  1. Use standardized scripts to deliver newborn hearing screening results to families/caregivers. Colorado EHDI is creating updated scripts that will be shared later this year. For now, here are some sample scripts.
  2. Communicate newborn hearing screening results to families/caregivers in their preferred language.
  3. Standardize the process for collecting family/caregiver contact information before the baby is discharged from the hospital or birthing center. Consider collecting contact information for a primary and secondary caregiver.
  4. Verify contact information for the baby’s healthcare provider before a baby is discharged from the hospital or birthing center. Ensure a release of records has been completed to share newborn hearing screening information.
  5. Counsel families/caregivers about risk factors for late-onset and progressive hearing loss.
  6. Schedule a follow-up appointment (rescreening or diagnostic hearing evaluation) before the baby is discharged from the hospital or birthing center.
  7. Provide written information to families/caregivers wherever possible. COEHDI Newborn Hearing Screening Brochure
