  1. Download and print out this form and send in a check payable to the Orangetown Jewish Center with BOR on the memo line.
  2. Complete this online form. You can then either pay online or send in a check for payment. Please note that there is a 3% processing fee plus a $0.30 cent transaction fee per online payment.
Either way, please submit your form and payment no later than Friday, June 24. No tributes can be accepted without payment.
