By July 15: Share Your Health Equity Insights
Are you or your practice involved with any formal community or cross-sector projects aimed at reducing health disparities by addressing social determinants of health? We want to hear from you!

Senior leaders from across Tufts Medicine are working to define a systemwide health equity (HE) strategy. This will be carried out in stages. The first stage involves doing an inventory of current HE-related initiatives across our system. This will help Tufts Medicine leverage expertise and resources, prioritize needs, and celebrate progress. We define “health equity initiatives” as any formal community or cross-sector project aimed at reducing health disparities by addressing social determinants of health. If you are involved in any HE-related initiatives, please take a few minutes to complete the HE Inventory Survey by July 15. For questions contact rodrigo.monterrey@wellforce.org.
Please join us at one of four webinars to learn about participation in two state and federal payor programs with July 15 and August 4 participation deadlines. Time also will be spent answering questions you may have.
