Community Care Physician Network
A Bridge to Success in Medicaid Transformation
Community Care Physician Network (CCPN) is CCNC's Clinically Integrated Network that will link practices to the valued Complex Care Management and Provider Services CCWNC is known for today. CCPN membership meets the requirements of the highest tier for Advanced Medical Homes (Tier 3) . Attestation begins October 1.

serving the Medicaid population of WNC.

Making a Difference for Patients and Providers
CCWNC is a proud partner in the MATCH program, McDowell County's KBR-funded, local health solution that uses clinical-community partnerships and case management to meet whole-person needs. A new 3-minute video shares the impact of the program through the eyes of a participant, partner, and local doctor.
CCWNC’s Quality Improvement team works closely with providers to help them get the most out of their EHR. Together, CCWNC QI Specialist and a team at the Henderson County Department of Public Health have been able to highlight the healthcare needs and program results that drive their services.
Behavioral Health Integration:
New billing codes for Psychiatric Collaborative Care Management (CoCM) allow NC Medicaid providers to integrate care management and psychiatric consults into primary care.

Virtual Health Roll-Out:
VirtualHealth Core Trainers are busy bringing the new care management platform to our network Champions! CCWNC team members have stepped up for user-testing, workflow development and peer assistance with in-house trainings fully underway.

Upcoming Events
