Community Care of WNC Celebrates 20 Years!
Our new brand roll out comes 20 years after North Carolina piloted the initiative that was then called Access II Care, an innovative population health approach to improve access to primary care medical homes and the quality of health care for Medicaid members. In this month's newsletter, read more about our history as we celebrate this milestone year, as well as our current work supporting patients, our Practice Transformation Network successes, and recaps of useful updates for providers across the health continuum.
Leaders Reflect on History and Value of CCWNC
Dr. Harald Kowa, founder of Blue Sky Pediatrics and current Board Chair for CCWNC, remembers the healthcare landscape in the early nineties as a time when patient care was disjointed and ineffectively managed, but when providers were already saying, “there has to be a better way.” 

Physicians and many motivated stakeholders were organizing during that time to consider disease management models and community protocols that would address outcomes for conditions like asthma and diabetes. However, these programs did not “come off the shelf” until 1998 when Access II Care, an innovative state pilot to recruit and support Medicaid providers, took root in the area. With its integration of Care Management into the model, this was when, as Dr. Kowa puts it, “the disease management program flew into the community as real, active plans.” For him, it was an “A-ha” moment of realizing that “ having a team of Care Managers is how things actually get done.”

Making a Difference for Providers and Patients
CCWNC Care Management teams are connectors, listeners, advocates, educators, nurturers, and go-getters. The patients they serve are resilient, curious, persistent, resourceful, hopeful and so much more. Long-time Care Managers share how their patients make an impact on them.
Dr. Amy Marietta, Medical Director and Laura Ellington, BH Counselor of the Blue Ridge Health--Polk team were highlighted in this month's CCPN Newsletter for their strong work on Behavioral Health Integration with support from CCWNC's Practice Transformation Network .
