Dear Friends & Supporters,

 
Did you know that medical conditions and mental illnesses often overlap?  For example, many people develop depression after a heart attack.  Certain medications prescribed to treat schizophrenia or bipolar disorder can cause weight gain, high blood-pressure and diabetes.  Some medications prescribed to treat thyroid disease can cause anxiety symptoms. 

The average life span for individuals with co-occurring mental disorders and chronic medical conditions is 25 years shorter than the general population. Thousands of people with mental health problems and chronic medical conditions don't receive the complete comprehensive care they need - partly because accessing multiple providers in different locations can be challenging, and partly because the various providers don't record patient data in the same medical record for patients that they share.   

So what's the solution? The Commerce City Health Home - one of only four sites in Colorado that was awarded funding for bi-directional integrated service delivery through Colorado's State Innovation Model grant.  The Federal Center for Medicare and Medicaid awarded Colorado $2 million to achieve the Triple Aim of better health, better patient experiences and lower costs. Basically, it focuses on integrating behavioral and physical health in primary care medical homes.  In 2014, Community Reach Center and Salud Family Health Center partnered to provide a behavioral and medical health care home for the residents of the Commerce City community.   

The Commerce City Health Home is located within Community Reach Center's outpatient clinic in Commerce City.  A celebration of the Health Home opening was held May 17th and well attended by many Adams County elected officials and community partners.  The Health Home employs a diverse variety of about 60 medical, dental and mental health professionals in order to close the health care gap in Commerce City. Many of the providers are bilingual.  All Health Home providers who are involved in the treatment of the same patient have access to that patient's health record.  Locating all providers in the same clinic allows them to meet and discuss their shared patients so that the entire care team has a complete picture of each patient's healthcare needs and treatment plan.

The best news about integrated care provided at the Commerce City Health Home is a reduction in the duration, intensity and cost of treatment required.  The result?  Improved clinical outcomes, a far better health care experience and more value to the client and the community.

Recruiting Volunteer Peer Counselors for Senior Reach Program
  
Are you at least fifty years of age and looking for fulfilling volunteer work?  Are you a caring, compassionate person who especially enjoys the company of older adults?  Then the Senior Reach program at Community Reach Center needs you! 
 
Senior Reach is a community-based program that identifies older adults in need in-home emotional or physical support in their homes and/or connection to community services.  Responsibilities of the Volunteer Senior Peer Counselors include visiting with homebound seniors in the Adams County community, and attending on-going training with the program coordinators once a month.
 
Senior Reach will host a presentation to provide information about volunteer senior peer counselors on Friday, June 10, 2-3:00 p.m. at the Thornton Senior Center, 9471 Dorothy Blvd, Thornton, CO 80229.  After the presentation , participants who are interested in volunteering can complete paperwork to begin their application process.  The process includes an interview, a background check, and participation in 6 weeks of training.

Stomping Out Stigma!

A HUGE Community Reach Center THANKS to all who supported our Stigma Stompers team's fundraising efforts for the 5K NAMI Walk at Centennial Park on May 21st. Our team raised nearly $1400 - ranking us 15th out of 108 teams for fundraising!  Stigma Stompers' donations will be divided equally between NAMI Colorado and NAMI Adams County.

Mental Health First Aid
Space Still Available in the June 14th Adult Module Mental Health First Class!
  
Community Reach Center offers MHFA classes monthly - FREE of charge for anyone who lives or works in Adams County. Join the more than 20,000 Coloradoans currently certified in MHFA. 
  
Mental Health First Aid (MHFA) training is a fast-paced, dynamic class that teaches you to recognize risk factors and warning signs for a range of behavioral health problems - PLUS a five-step Action Plan to provide basic assistance to someone experiencing a mental health crisis.
  
The MHFA course is designed for the general public. The Adult Module covers depression, anxiety disorders, psychosis, and substance use disorders.  The Youth Module is for anyone who is involved with adolescents and covers various mental health challenges common among youth 12 to 18 years of age.  All modules teach the evidence-based five-step Action Plan.
  
Parent Workshop:  How to talk with your teen about substance use

  
The perfect workshop for parents or guardians committed to preventing substance use in their teens - and for those currently facing this issue who want to learn how to motivate and support behavioral change for their teen.
