Welcome to the first edition of LHM Foundation's monthly newsletter, The Beacon!


As I reflect on 2024 – LHM Foundation's first full year! – I feel blessed by the opportunities our team has had to serve others, energized by the work to come, and grateful for the support of our donors, volunteers and community partners.


Although we enjoyed many bright moments in 2024, two stand out as springboards for what's to come in 2025:


  • Late last summer, our hospice team achieved accreditation through the Accreditation Commission for Health Care. ACHC accreditation is a testament to our ongoing commitment to providing exceptional care to patients and their families as they navigate the end of life journey.


  • In November, we announced our new transitional housing program, Horizons, and the Expanding Horizons fundraising campaign. The Horizons program will provide a vital resource to individuals and families facing homelessness in our area, giving them a safe place to live and supportive programming designed to help them overcome their barriers to housing security.

VOLUNTEER SPOTLIGHT

WE NEED YOU!


Volunteers are the key to our work. Whatever your interest, we have a volunteer role for you!


  • Hospice volunteers care deeply for our patients and their families. Volunteers can serve in direct patient care roles (e.g. providing companionship or doing light housework) or in administrative roles in our offices.


  • Beam Team volunteers help us to build relationships with other local nonprofit organizations by serving lunch at shelters, packing groceries at food banks, organizing donations, and more! Beam Team volunteers will also play an important role in our Horizons transitional housing program.

Planning is in full swing! In February, LHM Foundation will begin transforming a former senior living facility into the home of the Horizons transitional housing program.


As we announced in November, our 42,000 square-foot family care facility will feature 58 resident rooms for individuals and families facing homelessness, as well as a commercial-grade kitchen, gathering areas and more, all on almost 15 acres of green space! Horizons participants will engage in programming on topics such as financial literacy, educational advancement and job coaching, while living in an environment that encourages spiritual, emotional, and psychological growth.


We will be hosting open houses at our family care facility in the coming months. Watch for dates on social media and come out for a visit!

LHM Foundation has embarked on its Expanding Horizons campaign with a goal of raising funds sufficient to open its family care facility to homeless individuals and families in South Central Pennsylvania by October 2025. A generous donor is matching dollar-for-dollar the first $3 million raised!

Celebrating our one-year anniversary! LHM Foundation's hospice team began caring for patients and their families just over one year ago, on January 10, 2024. Every day since then, our clinical team has embraced the honor of walking alongside those navigating the end-of-life journey.
