Health IT Success Story:
Cape Cod Healthcare
In 2016, MeHI awarded Cape Cod Healthcare (CCHC) $430,000 for a two-year project to connect to the Mass HIway and manage the electronic transfer of clinical information between hospitals and community health centers or post-acute care providers whenever a patient is discharged. The project addressed many barriers involving clinical documentation standards and direct messaging. Looking forward, CCHC is expanding the project to other organizations throughout Cape Cod, allowing CCHC to access real-time medical information for all patients immediately upon admission. Learn more about CCHC’s success under MeHI’s Connected Communities Implementation Grant Program.
