Diabetes prevention classes and materials available to patients
More than 84 million Americans having prediabetes and 90 percent don't know it, according to the Centers for Disease Control and Prevention. It's critical to educate patients on symptoms and tips to preventing diabetes.

A diabetes prevention program can help at-risk patients learn about and commit to lifestyle changes. Many programs are free to patients. Click here for a list of local diabetes prevention programs occurring in the first quarter. Practices or patients should contact the program coordinator directly to confirm dates and locations.

The Physician Alliance created a variety of materials relating to diabetes prevention and management, including exam room posters, action plans and patient self-management forms. These materials are complimentary to TPA practices - click here to view.
Patient education materials help initiate important conversations
The Physician Alliance created the a variety of patient education marketing materials for our practices to use to stimulate conversations with patients. These materials can help with completing screening and closing gaps in care.

Practices are encouraged to display materials in waiting areas and exam rooms, and share with patients. Materials can be downloaded from TPA's website or select materials can be ordered via an online order form . The patient education marketing materials are available complimentary to The Physician Alliance member practices.
