Many resources available to help practices close gaps in care
Closing gaps in care takes consistent work but the efforts can help improve patient care and quality metrics. The Physician Alliance offers many resources to assist member practices with this important work.

A gaps in care plan should include:

  • HEDIS gaps in care reports generated through a practice's electronic medical record and through TPA's disease registry, Wellcentive.
  • running reports using metrics on a tracking tool (provided by your practice resource team (PRT) member)
  • reminder letters to patients listing screenings and tests that are due; follow up with patients via phone calls, texts, emails
  • utilizing patient portals for additional reminders
  • a copy of worked reports and lists in your patient centered medical home binder, along with the tracking tool to go over at PRT visit

A Gaps in Care Toolkit is available for members that includes a letter template to send to patients, tip sheet for closing gaps, patient education flyers focused on a variety of quality measures, social media graphics to promote cancer screenings, diabetes management, retinal eye exams and other services, and more.

3 items a practice website needs to increase user engagement
Physician practice websites are a valuable resource for patients and healthcare providers, but how to create an effective one? There are three necessary components for an effective practice website.
Personalized Content:
People have a lot of options available for getting information. Increase user engagement by ensuring a website is updated with the latest content. Stale information on a practice website should be updated with timely and personalized content. Share information on services, providers, accepted insurance plans, patient forms and other content relevant to the practice. Make sure practice contact information and location are also posted on the website.
To get precise messages across in the medical field, don't recycle old content or provide anything that doesn't add value! Personalized content helps readers feel like they can trust the expert.
Practice websites need to have a Secure Sockets Layer (SSL) certificate. The SSL is a digital security protocol that encrypts the connection between the site server and end user. Information such as credit card numbers, social security numbers, or other sensitive data are encrypted during transmission from the user to the practice website. This is the simplest way to build trust with users and should be the very first step when creating a website.
Search Engine Optimized Keywords:
With constant changes in Google algorithms, it's important to make sure that a practice’s website is optimized for success! One way to accomplish this is by using search engine optimized (SEO) ranking keywords. Identify what words people are searching for with regards to services and then place the words very strategically throughout the website content. This will help the website rank higher in searches than the competition.
