An astonishing 84 million Americans (1 in 3) have prediabetes and
are at higher risk of developing Type 2 diabetes. Nearly 90 percent of people with prediabetes don't know they have it or that it can be reversed.
The Physician Alliance is working with practices to focus on diabetes prevention through education and screening of patients. TPA is in the process of creating letter templates, tip sheets and patient education posters and handouts. In the meantime, the American Diabetes Association, American Medical Association and Centers for Disease Control and Prevention created an easy to follow
diabetes risk assessment. This form can be printed and distributed to patients to determine their risk factors. It's a helpful tool to initiate conversations with patients on diabetes prevention.
