Celebrating 40 Years of Leading the Change
   
The CarePlus Foundation will hold its 20th Annual Courage Awards Gala on Wednesday, June 6, 2018 at the Venetian in Garfield, to benefit the clients and programs of CarePlus NJ.  This year, Joseph Masciandaro is being honored for his exemplary leadership and service at CarePlus for the past 40 years, setting the standard and raising the bar for excellence and compassion in behavioral healthcare.

"There are no words to express the important role that Joe Masciandaro has played in the development of the community mental health system throughout the years," explained Mary Ann Uzzi, co-chair of the 2018 Courage Awards Gala. "He truly is a visionary. His dedication to advocacy exemplifies his commitment to comprehensive and compassionate care."
 
Over the four decades under Masciandaro's leadership, the organization has impacted countless lives. As an early provider of community-based services to the more severely ill, Mid-Bergen eased the process of deinstitutionalization. Soon after Masciandaro became Executive Director, the organization began serving those dually-diagnosed with mental health and substance abuse disorders.

With the official name change to CarePlus, the organization set a new standard for a complete continuum of care. The evolution continues to this day, with new grants continually being pursued and awarded, allowing for the creation of programs that fill the gaps within the system.
 
Proceeds from the Courage Awards are dispersed to those enrolled in CarePlus services through a variety of channels, including: educational scholarships, housing and medication assistance, substance abuse treatment, outpatient services and therapy for children and adults, as well as advocacy and community education.

CarePlus Shifts the Paradigm for Schools
Therapeutic  Day School to Address Behavioral Health Needs

Schools throughout Northern New Jersey have been contracting with CarePlus to bring evidence-based curriculum and clinical support to address the needs of their students. The services have expanded to 16 districts, not including those that utilize CarePlus for crisis response and school clearances. After doubling clinical presence in schools throughout the region, leaders of the CarePlus School-Based Program are in the process of opening the Paradigm Therapeutic Day School (" Paradigm") in July 2018.

As an extension of the mission of CarePlus, the Board of Trustees for the CarePlus Foundation approved a loan to cover the start-up costs for Paradigm. The school will serve students, who range in age from 14-21 years, who have been classified by their school districts as having behavioral and emotional disabilities.

"We are elated that the CarePlus Foundation is supporting us in turning the vision of Paradigm into a reality. Many students seeking an alternative have struggled for years with academic, social, and emotional challenges, giving them a sense of hopelessness in school," explained Tara Augustine, COO at CarePlus and CEO for Paradigm. "We are responding to this growing need, to complete the continuum of care and best serve those students with significant mental, emotional and behavioral health issues."

Administration is committed to transitioning students back to sending districts with the skills needed to be successful, which is achieved through close collaboration with district Child Study Teams. The goal is to establish a supportive and "learner-centered" environment that addresses the student's entire academic, emotional, and life skills needs. By taking an integrated approach to their care, the program will generate sustainable behavioral growth based on the student's intrinsic motivation to succeed.

"As a subsidiary of CarePlus, Paradigm has the added advantage to go above and beyond typical counseling standards," stated Jeffery Kahn, Executive Director and Principal at Paradigm. "What we are doing is different from anything that is out there."

Kahn went on to explain that the local school districts' response to the state required "needs assessment" was overwhelming. The state required a minimum of 24 students referred by districts for the school to open, and Paradigm received 70. Kahn noted that this indicates significant need and support for the school, and such enthusiasm is a reflection of the respected reputation and reliability of CarePlus in the local communities.

 
 

Update on Substance Use Grants & Programs
So Many Acronyms, Where to Begin?

Since the beginning of the fiscal year, CarePlus has solely submitted and collaborated with community partners on a number of grants to extend and streamline services for individuals and families in our community.  If you or someone you love is struggling with addiction, please take note of these new entrance points to access care and be connected with the most appropriate level of treatment.

HART (Heroin Addiction Recovery Team)
The HART program was launched in May 2017. This innovative and preventative approach to addressing the opioid epidemic established an open door at these local police stations. Individuals and families struggling with addiction can walk in and be connected to an addiction specialist for information, assessment, referral, resources and treatment plans. Follow up services are also offered once an individual is linked to treatment.
 
SUN (Substance Use Navigator)
On September 20, 2017, Care Plus NJ was selected by the NJ Department of Children and Families to implement the Substance Use Navigator program in Bergen County. CPNJ currently employs two Substance Use Navigators that are part of a state wide network tasked with identifying community needs and furthering the community's access to substance use resources for youth under 21 years of age.

STAR (Support Team for Addiction Recovery)
On October 6, 2017, the application submitted on behalf of Care Plus NJ and Children's Aid and Family Services was selected for the Support Team for Addiction Recovery (STAR) program. This DMHAS-funded program will provide case management and recovery support services for individuals with opioid use disorders in Bergen County. Care Plus NJ is providing two case managers for the STAR program, while Children's Aid and Family Services is providing two Recovery Specialists as part of this interdisciplinary team. 

FSC (Family Support Centers)
On February 26, 2018, Care Plus NJ was selected by DMHAS to implement the Family Support Center program in the Northern NJ region. This program will provide parent support, education, resources and advocacy to parents of young adults struggling with their loved one's opioid use disorder. Care Plus NJ has partnered with the Family Support Organizations in Bergen, Essex, Hudson, Morris, Passaic, Sussex and Warren Counties to provide ongoing group and individual services.

Outpatient  Treatment & Ambulatory Detox
We are currently licensed by the Department of Health for primary care services and by NJ DMHAS for our substance abuse services, including ambulatory detox, intensive outpatient care, and standard outpatient treatment. Our ambulatory detox services are specifically offered through our Certified Community Behavioral Health Center designation.


 
CCBHC's Demonstrate Outcomes & Hope
Demand Decision Makers to Follow Through

In July 2017, New Jersey was one of eight states selected to pilot a two-year federal demonstration project called the Certified Community Behavioral Health Clinic (CCBHC) program. The term "federal demonstration project" implies a trial period; an opportunity to experiment, to prove or disprove a hypothesis. In the case of the CCBHC's, the educated guess is that comprehensive integrated healthcare will reduce costs and improve outcomes for individuals with behavioral health conditions. The hope is that, by demonstrating useful outcomes, the project can be extended and expanded to serve more individuals throughout the nation.

A variety of factors need to be addressed in order to effectively treat these conditions. The CCBHC model has responded by establishing a one-stop shop for necessary services. CCBHC's are funded through a prospective payment system, which allows for coverage of activities which historically have not been included for reimbursement, such as services provided outside of the organization, telehealth services, peer services, and other essential social supports.

As one of seven New Jersey agencies selected to pilot the program, and with the experience of having one foot in each world, CarePlus can assert the benefit of the CCBHC and the harm of fee-for-service (FFS). Having taken every measure to reduce the loss, the CarePlus outpatient department still has a projected annual deficit of $2.1M due to the FFS transition. While the CCBHC bulk funding helps to offset that loss, it is a critical problem for which most agencies do not have a solution. New Jersey providers have warned about the dangers of FFS, and perhaps that helped to inspire this federal demonstration project... which is proving an effective route around the systematic traffic jam caused by the FFS funding transition.

Strengths that have been demonstrated through the CCBHC model include the ability to measure and address population health; reduce wait times for services; increase access to specialized services and evidence-based therapies while monitoring transitions in care from hospitalization to outpatient services. Perhaps one of the most promising developments from the CCBHC project is the licensed ambulatory detox clinic, completing the continuum of care for addiction in a community-based setting. There has been a lot of talk of expanding and improving substance abuse treatment options under FFS, but it is critical to realize that this is being done at the cost of mental health services.

The CCBHC test drive is well underway, and the vehicle is delivering as promised. Not only that, but it's important to point out that the alternative, the FFS model, is stalling the system. Now it's a question of whether or not decision makers will invest wisely by extending and expanding this data-driven model. CCBHC's are providing hope for the future of integrated behavioral healthcare, making tremendous efforts and producing promising outcomes ( see the report provided by the National Council).

It is time for decision makers to follow suite and overcome the seeming hopelessness for our national healthcare system by working with the facts clearly demonstrated by this federal project. In solidarity with the National Council for Behavioral Health, the New Jersey CCBHC's are uniting in their efforts to advocate for a one-year extension and 11-state expansion, as delineated in the Excellence in Mental Health and Addiction Treatment Expansion Act (S. 1905/ H.R. 3931).
