Growing through Referral Management: How to Put Patients & Families First, While Reaching Important Performance Goals

Even though many home care and hospice organizations are not satisfied with their referral management results, they may have difficulty seeing opportunities to improve these activities based on their team's varied roles and close proximity to the referral to admission process.  Referral management is an important part of daily operations that requires strategic staffing, training and ongoing tracking to create lasting changes that will help home care and hospice organizations grow.

William J. Simione Jr., Founding Member, Simione Healthcare Consultants, and  Robert J. Simione , Managing Principal, were each honored with the  Lifetime Achievement Award at the National Association for Home  Care and Hospice Annual Conference held in Orlando in October. The Simiones were lauded for their longstanding commitment to home care and hospice, their support of professionals in the field, and their efforts to guide the clinical and financial developments for improving the delivery of care in the home. Bob Simione was also honored at the Annual Conference of the Connecticut Association for Healthcare at Home earlier in October, receiving the  Friend of Healthcare at Home Award . The honor recognizes an individual, org anization or other group that has advanced the home care or hospice industry. 

Kathleen A. Hessler , RN, JD, CHC, CHPC,  Director, Compliance & Risk, w as appointed to the Texas & New Mexico Hospice Organization Board o f Directors. 

Anna M. Alvarez , RN, BSN, MSN, has joined the Simione team as a Senior Manager , supporting clinical consulting to meet the needs of clients involved in quality management and operations  improvement.  A  resident of Florida, Alvarez has more than 25 years of nursing and leadership experience in home care and hospice operations, consulting, rehabilitation management and acute care, including oversight of multi-site clinical operations and responsibilities ranging from quality and acquisitions to revenue cycle, cross-continuum program development, patient satisfaction and staff development.  Most recently, she was the Southeast Regional Vice President for Clinical Operations at Kindred at Home, Louisville, Kentucky,  where she led home health clinical operations for 62 branch locations.
