We're pleased to announce that the Sacramento Continuum of Care (CoC) will receive $6.5 million from the U.S. Department of Housing and Urban Development (HUD) to build a region wide plan and fund innovative programs designed to end youth homelessness. The funding is part of HUD’s $83.7 Million Youth Homelessness Demonstration Program (YHDP). It will come with federal technical assistance to develop and implement a coordinated community approach to preventing and ending youth homelessness. The first goal will be to develop the plan within six months and will include youth led collaborations. The CoC can then fund a wide range of housing programs including rapid rehousing, permanent supportive housing, transitional housing, and create host homes with YHDP funding.

The Homeless Housing, Assistance and Prevention Program Round 4 (HHAP-4) is a $1 billion block grant program authorized by AB 140, which was signed into law by Governor Gavin Newsom on July 19, 2021. This fourth round of state funding has been made available to CoC's, counties, and the 13 large cities to support building regional coordination and unified response to reduce homelessness across California. The HHAP-4 application template was released on September 30, 2022 and the application is due November 30, 2022. | |

On behalf of the Sacramento CoC, we are currently establishing the Housing Families First Collaborative (HFFC) and seeking membership. The CSH Medi-Cal Academy is a FREE web-based training series, available to homeless service providers throughout California, which will kick off in January 2023 and last through June 2023. CSH received funding from the California Health Care Foundation to provide free training and technical assistance to homeless service providers new to the health care space and new to contracting with MCPs. Any entities (providers and Counties) that are contracted or looking to learn more about providing housing-related CS are welcome. The Sacramento City and County Continuum of Care, City of Sacramento, and County of Sacramento recently pooled resources to invest $16 million to initiate the creation of a Coordinated Access System, aimed at ensuring people needing services have streamlined and clear paths to go through to access the right help. This investment will ensure that help is more equitable, expedient, and easier to find by our unhoused neighbors. System developments will include establishing a centralized hotline to quickly and equitably triage households to resources, including shelters, outreach, and housing assistance; a Coordinated Access Navigational (CAN) team to provide shelter system support and waitlist support; housing problem-solving (HPS) supports such as limited financial assistance to move more people to permanent housing.
Sacramento Steps Forward has been working closely with participating agencies, utilizing a phased approach to integrate the processes with shelters and 211 prior to public launch on December 5, 2022. In addition, Elica Health was recently awarded to establish the CAN team, and will be focused on hiring and developing a coverage plan with the CAS Core Development team. During this transition period, shelters and outreach partners can continue to submit requests for shelter by conducting the shelter assessment through the Homelessness Management Information System (HMIS) database. Please stay tuned for additional community-wide updates.

Sacramento County Public Health (SCPH), Folsom Cordova Community Partnership, Center of Praise, and Sacramento Steps Forward are facilitating a walk in flu and COVID-19 vaccine clinic on November 3 at Cordova High. People who receive their 1st or 2nd dose of the COVID-19 vaccine will be provided a $50 gift card. See the link for more clinics open to ages 6 months and older. Individuals are encouraged to visit MyTurn.ca.gov or call 1-833-422-4255 for more information.
Additionally, SCPH is monitoring monkeypox cases and tracking monkeypox vaccine clinics available to you via appointment only and walk in. However, only individuals who meet this criteria are currently eligible.

Sacramento Steps Forward (SSF) was selected by The Center at Sierra Health Foundation to continue its important COVID-19 outreach and education among people experiencing unsheltered and sheltered homelessness. The COVID-19 Community Health Project (CCHP) 3.0 award in the amount of $150,000 has been committed to SSF to serve community members across Sacramento County through interactive community outreach, trainings, and events as well as education focused engagement activities.
While SSF has been selected as a continuing partner in this statewide public health campaign, administrative hurdles have hindered the official project launch. We’re simultaneously proceeding with scaled back preparations, including connecting with recommended partners for community outreach and engagement. Our team hopes to provide additional details in the coming weeks. We greatly appreciate your support and patience—especially partners who have indicated interest in continued collaborations.
