Are you a pharmacy who is a recognized DPP or interested in stating a DPP? Or maybe just a pharmacy who is interested in supporting a DPP? Join our first NC DPP Pharmacy Learning Collaborative. Goals of the collaborative will be to connect pharmacies to discuss how to support patients with prediabetes, barriers and successes of offering the DPP, and more! Resources and training opportunities will also be shared during future collaboratives meetings.

Achieving Participant Goals and CDC Full Recognition

This Achieving Participant Goals document provides keys to success. Participant achievement of program goals (e.g., weight loss, physical activity, HbA1C reduction) for risk reduction and the ability of an organization to meet DPRP requirements for CDC full recognition are closely linked. Meeting participant goals is critical for improving participant outcomes as well as sustaining your type 2 diabetes prevention program long term. Programs with a proven record of meeting these goals and requirements have shared tips and lessons learned from their experiences. Review each of the tips to identify those that are most applicable to your program and participants.
