Tobacco Affidavit and Wellness Form.

Please take the time to complete the Tobacco Affidavit/Wellness Incentive form on E-Forms before October 24, 2025.

If you select option 2 on the tobacco affidavit, you are required to attach the tobacco certificate of completion to submit the form. We will receive a list of the participants who completed the tobacco cessation program. Any tobacco surcharges incurred in 2025 will be REFUNDED in the month of December 2025.

If you are unfamiliar with Neogov, I have attached instructions to complete the forms and to complete the tobacco cessation sessions. If you do not have a username/password or forgot your username/password, please contact the Human Resources Employment Team (Kelly Denham, Raven Williams, George Marshbanks) at 706-653-4059 to assist.
