PACE Case: Falls Assessment
After falling three times in August, participant AV reported three more falls in October, one of which resulted in a major injury for which she required surgery. The PACE Medical Director then requested a Falls Assessment from the CareKinesis clinical pharmacist. While not one specific medication on AV's profile was obviously problematic, the CareKinesis pharmacist did have medication safety recommendations for the PACE participant.

NPA Conferences
