The volume of resources can be overwhelming, and we want to make it easy for you to find our most popular tools. So each month, we'll highlight a favorite resource from an HHQI team member or a campaign participant.
Health literacy is an issue for 88% of U.S. adults, and it's even more prevalent among those 65 and older. In observance of Health Literacy Month,

shares one of her favorite HHQI tools that helps patients—like her father—keep track of important health information.

My Personal Health Record
is a paper-based, patient-friendly tool that can be used to keep all important patient information such as providers, diagnoses, surgeries, medications, etc. It's a 6-page tool that can be printed double-sided and folded in half to create a convenient booklet to take to medical appointments or to the hospital. .)
I use this tool all the time with my father; we never go anywhere without it. It saves valuable time for providers and helps me remember dates of his surgeries.

Immunization & Falls Prevention Updates
|
|
The HHQI team is currently working to update the
Immunizations & Infection Prevention Best Practice Intervention Package
(BPIP) to include the vaccine recommendations for the 2018-19 influenza season from the Advisory Committee on Immunization Practice (ACIP). This update will be available in
late-October 2018.
Our
Falls Prevention BPIP
is also being updated and will be posted in
November 2018
. In addition to the BPIP update, we're also developing a corresponding HHQI University course so you can earn free continuing education credits!

Promoting Medication Adherence among Older Adults at Home

Many patients have medication adherence and other issues that relate to physical, cognitive, or health literacy impairments. Such issues can affect the management of their medical conditions. Occupational therapists are taught evidence-based management interventions that help patients improve adherence and medication outcomes. This webinar is appropriate for leaders, nurses, therapists, and social workers who want to learn more about medication management strategies and the utilization of occupational therapists.

This event will feature a panel of patients living with pain, a pain management specialist, and a patient advocate who will explore different perspectives on pain management strategies.
